Procedure Flow

First Visit

With Appointment

Put the following documents in a pink folder and drop it into the box in front of Reception No.1 (First Visit Reception Desk).
After 11 o’clock, please go to Reception No.2 (Revisit Reception Desk)

  • Appointment Slip
  • Letter of Referral
  • Japanese Health Insurance Card
  • (If you have) Medical Care Certificate: Infant Medical Care Certificate, Eligibility Certificate for Ceiling-amount Application, or Elderly Recipient Certificate, etc.
  • Electronic Files of X-ray Images, etc. (CD-R, DVD, etc.)

Without Appointment

Fill in an Initial Visit Registration Form. Put the following documents in a yellow folder and drop it into the box in front of Reception No.1 (First Visit Reception Desk).
After 11 o’clock, please go to Reception No.2 (Revisit Reception Desk)

  • Initial Visit Registration Form
  • Letter of Referral
  • Japanese Health Insurance Card
  • (If you have) Medical Care Certificate: Infant Medical Care Certificate, Eligibility Certificate for Ceiling-amount Application, or Elderly Recipient Certificate, etc.
  • Electronic Files of X-ray Images, etc. (CD-R, DVD, etc.)
    You will be called when your medical chart is ready.

Revisit (with Patient ID card)

With appointment

①Insert your Patient ID card into the Registration Kiosk for Revisits

②Touch the ”Confirm”(”上記の内容で確認”) button on the screen

③A Registration Ticket will be printed out. Put it into a registration ticket folder

④Submit it to the reception of the department which you are visiting

  • *Registration Kiosks for Return Visits are located near elevators on each floor of the Outpatient Consultation Building. You may register with any of the machines.
  • *You will need your Patient ID card at the reception. If you have forgotten to bring it, come to Reception No.2 (Revisit Reception Desk) on 1F or consult the outpatient department counter you are visiting.
  • *If you have made your appointment from another medical institution and have an appointment slip, come to Reception No.1 (First Visit Reception Desk) on 1F and do not use the Registration Kiosk for Revisits.

Without appointment

You need to register for consultation. Come to the Reception No.2 (Revisit Reception Desk) on 1F.

Caution!
Even if you have visited our hospital before, you may be considered a first visitor when you meet the following criteria:


If so, you will be received at Reception No.2 (Revisit Reception Desk) and charged first visit fees of ¥5,500 (medical) or¥3,300 (dental).

  • You used to be treated for an illness but have now been cured of it
  • You have been away from this hospital for three months or longer, without specific instructions from your doctor to do so.

At Outpatient Department

Doctors will see patients by appointment. However, the scheduled appointment time is only an estimate. Please understand that patients with severer conditions may be prioritized.
Please talk with your doctor to schedule the next appointment.
Follow the instructions given by your doctors and nurses for examinations and laboratory tests.
After consultation, please receive a clear folder for payment.

After Consultation

Medical and dental consultation fees must be paid separately.

If you are not required neither to pay anything nor collect a prescription, you can go home after the payment calculation.

After a Medical Consultation

Please have your medical service fees calculated at a Payment Calculation Desk (1st-4th Floor).

Take a waiting ticket from the dispenser at the desk.
Please proceed to the desk when your ticket number is called, or the digital display board shows a number higher than yours.
You will receive the following documents at the desk.

  • Notice of Medical Service Fees
  • Appointment Slip
  • Prescription for External Pharmacies
  • In-hospital Medicine Exchange Ticket
  • Medical Billing Statement

* If you have received a medical report or certificate directly from your doctor, please present it at the Payment Calculation Desk.

After a Dental Consultation

Please have your dental service fees calculated at a Payment Calculation Desk (3rd-5th Floor) in Clinics for Outpatients. After the consultation, please wait near the department reception. You will be called and receive the following documents.

  • Notice of Medical Service Fees
  • Appointment Slip
  • Prescription for External Pharmacies
  • In-hospital Medicine Exchange Ticket
  • Medical Billing Statement

* If you have received a medical report or certificate directly from your doctor, please present it at the Payment Calculation Desk.

Payment (Self-service Payment Kiosks: 1st Floor of Clinics for Outpatients)

Please pay your medical service fees at a Self-service Payment Kiosk using your Patient ID Card, or you can make a payment at Reception No.6 (Payment Reception Desk).
We accept cash, debit and credit cards (JCB, VISA, MasterCard, American Express, J-Debit).

Prescription Drugs

Out-of-hospital Prescription
Please purchase the prescription drug at an External Health Insurance Dispensing Pharmacy.
Please ask our staff at Prescription FAX Station if you would like to transmit the orders in advance to an external pharmacy by fax.

Prescription FAX Station

In-hospital Prescription
If you receive an In-hospital Medicine Exchange Ticket at your outpatient department, go to the In-hospital Pharmacy Counter (East Ward 2nd Floor). When your medicine is prepared, your prescription number is shown on the electronic signboard. Present the ticket at the counter and receive your medicine.

Medical Service Fees

~Eligibility Certificate for Ceiling-amount Application (For patients under 70 years of age)~
You are exempt from paying the excess beyond Ceiling-amount of Personally-borne Medical Expenses (See the table below) by presenting this certificate and your Japanese health insurance card. Early application is recommended as “The Date of Issue” will be the first day of the month you apply.

  • *Ceiling Amount of Personally-borne Medical Expenses are calculated separately by consultation month, medical institution, medical/dental, and outpatient/inpatient.
  • *In principle, the ceiling amount is applied from the day you present the certificate. Please make sure to present it at the reception desk.
  • *Uninsured expenses such as documentation fees are not eligible for High-cost Medical Care Benefits. You must pay the fee at your own expense.

Receipt

A receipt is required for various certifications, such as a medical expense deductions. Please keep it safe as it will not be reissued. (If you have lost your receipt and request a proof of payment, you will need to pay an extra fee.)

Medical Certificates

Please request a medical certificate at Reception No.5 (Medical Certificate Reception Desk).

To Request

Please complete the application form and submit it with the designated format of the medical certificate. The documentation fee occurs at the reception. Please make a payment at a Self-service Payment Kiosk or Reception No.6 (Payment Reception Desk). If you would like to receive your certificate by mail, you will be asked to pay a sum for the postage of 1,570 yen.

To Receive

Please allow two weeks or more for processing. We will call you by request when the certificate is ready to pick up. Please receive it at Reception No.5 (Medical Certificate Reception Desk).

If you live a long distance away from our hospital

We also accept a request by mail if you live too far to visit. Please send the completed Application Form for Medical Certificate (PDF) and the format designted by the organization. (If you are unable to print out the application form, please write the same information on a piece of paper or request the application form by phone.) It will cost 1,570 yen for postage.
(Please understand that we do not accept a return envelope in order to ensure a secure delivery.)
